About Community Information Exchange and Coordination of Care

The CIE® connects health, social service, housing, and emergency partners through shared data and coordinated referrals, transforming traditionally disconnected systems into a unified network that can respond to individual needs in real time. By sharing a secure electronic database of resources and client information, CIE Partners can provide streamlined support, including referrals, eligibility screenings, and follow-up to ensure individuals are able to access the services they need. Real-time data allows communities to receive faster, more informed, and seamless support when it matters most.

CIE Makes Prevention More Effective

An analysis led by 211/CIE San Diego and regional partners shows how shared data and coordinated care through the Community Information Exchange (CIE®) significantly strengthens homelessness prevention efforts. By connecting 11 organizations through real-time, secure information sharing, partners were able to screen, refer, and support clients more efficiently and measure outcomes across the system. 


CIE’s shared infrastructure and integration with the region’s Homeless Management Information System (HMIS)—used by 80+ providers and 600+ programs—made it possible to track outcomes, reduce duplication, and ensure people accessed the services they were connected to. (SOURCE: https://ciesandiego.org/wp-content/uploads/2024/11/RTFH-Paper-10.7.24.pdf
 
The takeaway is simple: when organizations work together through CIE, resources go further, coordination is clear, and the community sees measurable impact. 

The CIE helps agencies focus on care delivery, helping clients stay enrolled, stable, and healthier, and prevent crises before they emerge by making data actionable and predictable.

The CIE® represents a community of Partner organizations that share an electronic database that contains resources and secure client information. CIE Partners offer clients support and care coordination by providing referrals, screening eligibility for additional resources, and following up on someone’s ability to access services.

Organizations that support San Diego and Imperial Counties are eligible to become CIE partners if they meet the Inclusion Policy and security requirements.

211 San Diego manages the CIE partnerships and technology. 211 maintains a database of more than 6,000 resources that serve San Diego and Imperial Counties. This is the same database that CIE Partners can navigate as a way to connect clients to services and refer clients to programs offered by partner organizations. When clients authorize CIE Partners to enter and share their information in CIE, Partners use the database to securely send and receive referral and eligibility information.

We believe access to health and social services provides a foundation for individuals and families to build and sustain healthy lives, so we’re collaborating with service providers in the San Diego region to build a more efficient, equitable, and accessible system that addresses the whole health of a person through integrated, person-centered care. Personal information is defined differently by various privacy laws. Within CIE, personal information includes identifiable information such as a full name, date of birth, phone number, email addresses, and other information disclosed by the client and other CIE Partners that supports providing effective treatment, care or support. Personalized Services: When 2-1-1 San Diego and other CIE Partners have access to basic information like contact information, household size, and primary language, it enables providers to match people to services that are likely a good fit for that person or family.

Direct Referrals: Some supportive programs accept electronic referrals from CIE Partners. This can expedite the screening process for services. Much of the time, people will receive follow up calls about their referral status and next steps. This can increase the odds of enrollment within services. More in one place: Seeking services can feel like jumping through hoops. Benefits applications and intake forms can be lengthy. After getting enrolled in one program, someone might end up with a list of 10 phone numbers to call for more support. That’s a lot to memorize or keep track of. When someone agrees to share their information with CIE, it brings much of that information to one place, making it manageable to know who to call, and what kind of resources are available.

CIE Partners are legally bound to only access personal information to support care coordination for the clients they are working with. Some examples of care coordination are activities such as using client information to coordinate referrals, screen eligibility for additional resources, and follow up on someone’s ability to access services.
